I. The test will be simultaneously held in Sabaragamuwa University of Sri Lanka and the University of Kelaniya. You are requested to fill the application only if you intend to apply for the test in Sabaragamuwa University of Sri Lanka.

III. Electronic version (image/ scanned copy) of following documents should be attached with the application.

1. Paying Voucher/ Receipt for Rs. 750/-
(Sabaragamuwa University of Sri Lanka Account No. 0002246976 at Bank of Ceylon).
2. Certified copy (Certified by the Principal or Justice of Peace) of G.C.E. (A/L)
2020 Certificate (Including Z Score & General test marks.)
